Just right click the image, and SAVE IMAGE AS to your desired location. Make sure it says .png, or it will not have the transparent holes. 

This is a PNG mask for the Tim Holtz Alterations, picture wheel die. It can be found just about everywhere, I highly recommend it! It is a Sizzix brand die, and you need a die cut machine to use it. The box is a .53x.58" box, that you clip your photo to, and tuck behind the picture wheel to place it. After you fill all the boxes, save the photo settings and remove the picture wheel template. You can print it on a piece of acetate, or any printable paper.  Save it as a PNG file if you use acetate, otherwise it doesn't matter if it is jpg or png.

This is not meant to be a tutorial, but I will say you need photoshop, or a similar program, and a printer. A die cutting machine, cardstock or chipboard, printable paper or acetate, adhesive, a brad or something to attach to the middle hole.
